Holep stands for Holmium Laser Enucleation of Prostate. Thulep stands for Thulium Laser Enucleation of prostate. Holmium and Thulium are different kinds of laser that can be used for surgery. For ease of understanding, some urologist will advertise these treatments as laser prostate surgery.
How this technique works is that, these laser machines produce a veru sharp light beam that can cut through tissue. Using this laser light, the urologist is able to cut out your prostate gland (somewhat like a coring an apple). Because the laser light is so strong, it causes no bleeding as it burns and seals all bleeding vessels at the same time while it cuts.
Though this technique is advantageous, it is currently recommended for large prostates or in patients with heart disease. This treatment is comparatively expensive, because the laser machines are expensive and the training to learn this surgery is expensive, but after the procedure most patients can resume their daily work immediately within a day or two, so the expense makes sense.

What is TURP?

Trans urethral resection of prostate or TURP is the old gold standard treatment of procedure. Economical with excellent results, this procedure is the best procedure for prostate removal. The price difference between turp and laser prostate can be a lot, so consult with your urologist regarding which treatment would be the best for you.
In TURP, instead of using laser, electric current is used to cut and burn away tissue, leading to complete removal of the prostatic tissue and giving you symptomatic relief.
The disadvantage compared to laser prostate is that you would require bed rest for 4 to 5 days longer, but after a week, most patients feel the same after any option.
The advantage is that TURP may cost half or even less of a laser prostate treatment. Some insurance providers may not reimburse you for laser prostate treatments.
What to expect with your prostate surgery?
For your prostate surgery you will need to be admitted. Though the surgery takes 1 to 2 hours to perform, you will need to stay in the hospital for a couple of days after the surgery. You will usually get admitted on the morning of surgery. The surgery will be done in spinal anethesia, that is, a small injection in your back will make you feel no pain below your waist. You will be awake and if possible will be able to listen to music or watch a movie while the surgeon operates.
After the surgery you will have a urine pipe or a foleys cahteter in your bladder for 2 to 3 days after which it will be removed and you will be allowed to pass urine.
For the next 2 weeks your flow will progressively improve. Some burning or pain may occur, but usually resolves in 1 or 2 days. You can usually start office in a week and your heavy work like gym in a month.
