WHAT IS ROBOTIC SURGERY?
Robotic surgery is not what it is made out to be. A robot does not operate autonomously as is projected. It is nothing like a self driven car.
Robotic Surgery is a specialised field of surgery in which a Slave system (the Da Vinci Robot) is controlled by a master unit (The console surgeon). The movement of the surgeons hands are replicated by the surgical robot inside the patient. A powerful camera provides 3 D High Definition view of the patient to the surgeon sitting remotely as he operates in a virtual environment and the exact movements are replicated inside the patient.
With the tools at his console the surgeon can perform intricate tasks such as cutting, stitching, moving and burning with very small incisions and great cosmesis to the patient.
Because the surgeons hands do not need to be placed inside the patient, small 8 mm incisions can accomodate the robotic arms inside the patient, thus saving the patient from scars and post operative pain.
WHAT ARE THE ADVANTAGES OF ROBOTIC SURGERY?
The advantages of robotic surgery for the patient are
- COSMESIS – DUE TO SMALL SCARS
- LESS BLOOD LOSS – BETTER POST OPERATIVE FEELING
- BETTER RESULTS WITH INTRICATE SURGERIES DUE TO HIGHER MAGNIFICATION
- EARLY RETURN TO HOME AND WORK
- THESE BENEFITS ARE STILL DEBATED IN SCIENTIFIC LITERATURE AND REAL BENEFITS ARE CONTROVERSIAL – CONSULT WITH YOUR UROLOGIST
WHO CAN PERFORM ROBOTIC SURGERY?
The robot is a specialised tool. It is a machine that can cause significant harm and complications if not used right. But as of now there is no legislation in India that asks for a specialised degree or qualification to perform robotic surgery.
Any surgeon trained in operating on a Da Vinci Robotic surgery platform, can use the robot for surgery. Though the results and complications improve with experience.
The makers of the surgical robots “Intuitive Surgical” expect all surgeons to train and complete certification before obtaining console rights.
Many senior surgeons and colleagues have years of experience and were the pioneers in the field. They received training abroad, before pioneering robotic surgery in India. It is their efforts that today Robotic Surgery training is available in India and is as good as anywhere in the world. We young robotic surgeons owe our gratitude to them.
Among these early pioneers are Dr AK Mandal, Head of Department, Department of Urology, PGIMER Chandigarh, whose far sighted vision enabled him to establish one of the best centres for Robotic Surgery in North India at my Alma mater. I had the good fortune of training for 3 years at this centre where I observed assisted and performed cutting edge Robotic Surgery.
He also developed a terrific team of Senior Surgeons and Consultants par excellence, whose expertise has been a source of Inspiration and learning. Among them I had the good fortune of learning with Dr SK Singh, Dr Santosh Kumar and Dr Ravimohan – each of them having years of experience in India and Abroad.
