What is the best time to get an arteriovenous fistula made?

The best time to get a av fistula surgery is two to three months before you are expected to begin hemodialysis. That would be when your serum creatinine is around 8 and eGFR is about 15 to 20. At this time your body is most likely to heal, complications are minimal and you will get two months for the fistula to mature.

Otherwise, earlier in the course of hemodialysis, earlier the better.
